Differential diagnosis and uncertainty

Ophthalmic findings frequently have more than one reasonable explanation. Content on this site that describes typical presentations, mechanisms, or causation reasoning necessarily simplifies; in any real matter, reasonable alternative diagnoses must be considered and the limits of the available evidence acknowledged. Where the evidence does not support a conclusion to a reasonable degree of medical probability, an honest evaluation says so.

Limits of records, imaging, and isolated tests

Photographs, imaging studies, and isolated test results have inherent limitations. Imaging can contain artifacts; automated testing depends on patient effort and cooperation; records may be incomplete or internally inconsistent. Sound analysis considers each finding in the context of the whole record rather than in isolation.

No physician-patient relationship

Nothing on this website creates a physician-patient relationship. Dr. Shomer does not provide medical advice, diagnosis, or treatment through this website or through the inquiry form. Individuals with eye symptoms should consult their own ophthalmologist; sudden vision loss, eye pain, flashes, or a curtain over the vision warrant urgent professional evaluation.

No expert-client relationship

Submission of an inquiry does not retain Dr. Shomer and does not create an expert-client relationship. Retention occurs only through a written agreement following conflict review.

Independence of opinion

Dr. Shomer accepts carefully selected plaintiff and defense engagements. His opinions are formed independently after review of the relevant facts, records, examination findings and applicable medical literature. He does not offer opinions that the evidence does not support, and retention by a party does not determine the content of any opinion.
